It Takes special
people to provide the hospice care you need.
Serving hospice patients and their families is a
privilege and requires one of the greatest amounts of trust you'll ever
place in a health care professional. We don't take that trust
lightly. Our trained nurses, volunteers and staff are ready
to help you through uncertain days ahead with sensitivity and
compassion. Hospice of Tennessee Valley's interdisciplinary
group of professionals and volunteers develop with each
patient/family/caregiver a Plan of Care which includes, as appropriate,
the following services and team members

Attending Physician, Medical Director, Registered
Nurse, Hospice Aide, Medical Social Worker, Homemaker Services, Grief
Counseling, Spiritual Support, Hospice Volunteers, Therapists
(contracted Physical, Occupational and Speech) Dietary Counseling
(contracted), Consultant pharmacist, Durable Medical Equipment,
Medications and Medical Supplies related to the hospice patient's
terminal diagnosis.
Inpatient Care:
Short term stay for symptom control and planned respite for
family/caregivers
Bereavement Follow-up for 13
months following death.
Continuous Care:
24 hour skilled nursing care provided in place of residence for short
term medical crisis care.
Grief Support Groups, lead by
our chaplains.
